This step begins with taking the NCD survey. After receiving your results, you will be analyzing your situation to determine the root causes of your minimum factor. Careful diagnosis, such as the process outlined in this resource, will allow the church to develop a plan that will increase their health (and score) by addressing the root causes and not just the symptoms. - Select survey participants.
- Introduce participants to basic NCD concepts.
- Complete the NCD survey.
- Score the questionnaires.
- Receive survey results and discuss the next steps with the NCD coach.
- Process initial reactions and responses (emotional tollbooth).
- Expand church health team in light of minimum factor.
- Develop a general process with timeline for each step of the NCD process: diagnose, plan, implement, evaluate.
- Conduct focus groups to discern key issues contributing to the minimum factor.
- Do additional reading and study on the minimum factor.
- Negotiate ongoing role of the NCD coach.
- Communicate progress and prayer needs.
- Present the survey results to the congregation (optional).
